Once a document is notarized, it may then be sent to the Secretary of the Commonwealth of Virginia for authentication if this level of authentication is needed. 1. Complete the NOTARIZED DOCUMENT REQUEST form.
2. Mail or drop off the document(s) to be notarized to the following address:
